Name the affected part of the site as well as the address. “The east dock, bays 14 through 16” identifies the opening. Include the gate, check-in or badging procedure when known; the street address alone may not identify the right entrance.
Report a pattern without diagnosing it. If bays 14, 15 and 16 have all started sticking, say that. A qualified provider can assess whether the faults share a cause or need separate repairs.

The building codes took effect June 23, 2026, and the International Fire Code took effect April 23, 2026, with the 2023 National Electrical Code. Two different effective dates in the same adoption is not a typo — the fire code took effect two months before the building codes. If your project touches a rated assembly, that matters.
And Goodyear is not Phoenix. Phoenix has been on the 2024 Phoenix Building Construction Code since August 1, 2025, with its own SHAPE PHX portal. Unincorporated Maricopa County is still on the 2018 I-Codes adopted August 17, 2022. Do not apply Phoenix dates, Phoenix amendments or the Phoenix portal to a Goodyear site. The base edition, local amendments and submission process must be checked for the parcel.
Goodyear Building Safety is at 1900 N. Civic Square, and the city's main line is 623-932-3910. Ask them three things: whether the parcel is inside Goodyear, whether your scope needs a permit, and which edition your project will be reviewed under.
Check the company’s commercial or dual license in the ROC lookup and ask which classification covers this door or dock work. New or altered operator wiring raises a separate C-11 or CR-11 scope question.
