Is it a dock plate, a dock board, or a leveler?
A dock plate and a dock board are both loose, portable bridges that get carried to the dock edge. A leveler is installed in the dock and stays there. Plates and boards are built for different loads, and the rating on the unit decides what it can carry.
One manufacturer puts the difference this way. A dock plate is "a flat piece of metal, either steel or aluminum, that will typically have one bend in it." A dock board "has the same features of the plate with the addition of side curbs." It adds: "Dock Plates are ideal for hand truck and pallet jack traffic," while "Forklift trucks, power pallet jacks and walkie stackers should be used with a dock board" (Bluff Manufacturing).

Nameplates. Bluff is one maker of plates and boards. Tell the provider the brand and rating only if you can already read them. A brand name alone never tells you what a unit can carry.
What OSHA says about dockboards
OSHA defines a dockboard as "a portable or fixed device that spans a gap or compensates for a difference in elevation between a loading platform and a transport vehicle," including "bridge plates, dock plates, and dock levelers" (29 CFR 1910.21). Its rules say:
- A dockboard has to be "capable of supporting the maximum intended load" (29 CFR 1910.26(a)).
- Portable dockboards are secured in place. They need handholds or another way to handle them safely (1910.26).
- Wheel chocks or sand shoes keep the transport vehicle from moving while people are on the dockboard (1910.26(d)).
- Dockboards put into service on or after January 17, 2017 must be designed to keep vehicles from running off the edge, unless there's no hazard (1910.26(b)).
- Workers on a dockboard need guardrails or handrails where they could fall 4 feet or more. The exception is motorized materials handling where the fall is no more than 10 feet and workers are trained (1910.28(b)(4)).
Idaho is under federal OSHA for private employers (OSHA State Plans). How these apply at your dock is a question for your safety lead.
What damage should you report?
Take a damaged plate or board out of use under your facility's process. Don't use it again until a qualified provider or the manufacturer has looked at it. Report what you can see without handling it more than you have to.

Damaged plate or board? Call and describe it.
Repair it, or replace it?
A web page can't approve a damaged plate or board to go back into service. That goes for welding, straightening and returning it to use. A qualified provider or the manufacturer decides, based on the unit's make, rating, the damage and the maker's instructions. Many damaged portable units simply get replaced.
Don't assume powered equipment is okay. A plate rated for hand trucks shouldn't carry a forklift, per the manufacturer quoted above. If the rating is unreadable, treat the load limit as unknown.
What a replacement discussion needs. The dock-to-trailer height range, the gap width, what crosses it (hand truck, pallet jack, forklift), how often, and the old unit's make and rating if already known. None of that is needed before you call.
When a leveler makes more sense. If a bay uses a portable board all day, an installed leveler may be worth asking about. Ask the provider about the bay and see dock levelers.

Can a dock plate carry a forklift? Don't assume so. One manufacturer says plates are for hand trucks and pallet jacks, and powered equipment should use a dock board. The unit's rating is what counts.
Is an edge-of-dock leveler portable? No. It's bolted to the dock. See dock levelers.
Can a cracked dock board just be welded? This page can't approve that. A qualified provider or the manufacturer decides. Keep it out of use until then.
